This article is the third in a series of four articles which extends the classification of subfactors to index 5. A comprehensive overview of the classification program can be found in the recent article [\textit{V. F. R. Jones, S. Morrison} and \textit{N. Snyder}, ``The classification of subfactors of index at most 5'', \url{arXiv:1304.6141}]. In Part 1 [Commun. Math. Phys. 312, No. 1, 1--35 (2012; Zbl 1246.46055)], \textit{S. Morrison} and \textit{N. Snyder} used the principal graph odometer and various principal graph obstructions to reduce the classification to a list of 5 weeds and 39 vines. A weed represents an infinite family of possible principal graphs obtained by translation and extension, and a vine represents an infinite family of possible principal graphs obtained by only translating. In Part 2 [Int. J. Math. 23, No. 3, Article ID 1250016 (2012; Zbl 1246.46054)], \textit{S. Morrison, D. Penneys, E. Peters} and \textit{N. Snyder} ruled out three of the five weeds, using various triple point obstructions and graph norm arguments. In this article, the authors rule out the remaining two weeds, using two new quadruple point obstructions. The first is planar algebraic and rules out one weed, and the second uses connections and cyclotomicity results of \textit{F. Calegari, S. Morrison} and \textit{N. Snyder} [Commun. Math. Phys. 303, No. 3, 845-896 (2011; Zbl 1220.18004)] to rule out both weeds. The authors then prove the uniqueness of the GHJ 3311 subfactor by analyzing the diagrammatic branch matrix, a powerful new tool obtained from combining connections and planar algebras. In Part 4 [Int. J. Math. 23, No. 3, Article ID 1250017 (2012; Zbl 1246.46056)], \textit{D. Penneys} and \textit{J. E. Tener} ruled out the vines, using the cyclotomicity results of Calegari-Morrison-Snyder [Zbl 1220.18004].
