The classification problem of the title is solved in all details over an arbitrary field \(k\). The essential ideas go back to \textit{W. C. Waterhouse} [Invent. Math. 37, 157-164 (1976; Zbl 0337.10015); Pacific J. Math. 69, 275-283 (1976; Zbl 0362.15015)]. Note that the classification of pairs of quadratic forms remains open for \(\text{char }k= 2\).
