Solution of the rank equation \[ \text{ rank}\begin{pmatrix} A & B \cr C & X \end{pmatrix} = \text{ rank}(A) \] for the unknown block \(X\) is discussed in the case where \(A\) is square and the index of \(A\) is 1. Conditions for the existence and uniqueness of the solution are given. If \((\;)^\#\) denotes the group inverse, and \(B=AA^\#\) and \(C=A^\#A\), then \(X=A^\#\) is the unique solution. On the other hand, if \(X=A^\#\) is a solution, then all the corresponding blocks \(A\), \(B\), and \(C\) are characterized. The results of \textit{Y. Wei} [Appl. Math. Comput. 98, 29--42 (1999; Zbl 0927.15004)] and of \textit{J. Groß} [Linear Algebra Appl. 289, 127--130 (1999)] are special cases.
