    In this paper a conserved phase field model with memory and relaxed chemical potential is proposed. The model is based on the linerarized Cahn-Hilliard theory of spinodal decomposition of binary mixtures and the Rostein et al. computation of the relaxing chemical potential in phase transitions with memory. Local existence and uniqueness under some regularity and growth assumptions for the potential function and for a certain class of kernel functions are proved. Global existence and uniqueness are also proved under stronger assumptions.
