    The authors complete the classification of 4-dimensional non-degenerate affine hypersurfaces with parallel cubic form with respect to the Levi-Civita connection of the affine Berwald-Blaschke metric. They prove the following main theorem: Theorem. Let \(M\) be a non-degenerate affine hypersurface in \(\mathbb R^5\) with \(\hat{\nabla}C = 0\), and suppose that the affine metric is of signature 2. Then either \(C = 0\) and \(M\) is a quadric, or \(C\not= 0\) and \(M\) is locally affine equivalent to one of the following ten hypersurfaces: {\parindent8.5mm \begin{itemize}\item[(1)] \(x_5=x_1x_2 + x_3x_4-{1\over 3}(x^3_1+ x_3^3)\); \item[(2)] \(x_5=x_1x_2 + x_3x_4-{1\over 3}(x_1x_3^2+ x_3x_1^2)\); \item[(3)] \(x_5=x_1x_2 + x_3x_4-{1\over 3}x_1x_3^2\); \item[(4)] \(x_5=x_1x_3 - x_1^2x_2+{1\over 4}x_1^4+{1\over 2}(x^2_2- x_4^2)\); \item[(5)] \(x_5=x_1x_2 - {1\over 3}x_1^3+{1\over 2}(x_3^2- x_4^2)\); \item[(6)] \(x_5=x_1x_4 + x_2x_3-x_1^2x_3-x_1x_2^2+x_1^3x_2-{1\over 5}x^5_1\); \item[(7)] \((x_2^2+x_3^2+x_4^2-x_5^2)^2x_1=1\); \item[(8)] \((x_3^2+x_4^2-x_5^2)^3(x_1^2+x_2^2)^2=1\); \item[(9)] \((x_2^2+x_3^2)(x_4^2+x_5^2)x_1=1\),; \item[(10)] \((x_3^2+x_4^2+x_5^2)^3(x_1x_2)^2=1\). \end{itemize}}
