The main result is the following theorem: Let \((M,g)\) be a pseudo-quaternionic Kählerian manifold of signature \((4,4)\) with nonzero scalar curvature. Suppose that the holonomy algebra \(g\subset sp(1)\oplus sp(1,1)\) of \((M,g)\) preserves a 4-dimensional isotropic \(I_1,I_2,I_3\)-invariant subspace \(W\subset\mathbb{R}^{4,4}\). Then \(g\) is conjugate to one of the following subalgebras of \(sp(1)\oplus sp(1,1)_{HP}\): \[ sp(1)\oplus sp(1,1)_{HP},\quad h_0= sp(1)\oplus \left\{O_p\begin{pmatrix} a & 0\\ 0 &-\overline a\end{pmatrix}\mid a\in\mathbb{H}\right\}. \]
