The authors show that there are precisely two, up to conjugation, anti-involutions on the algebra of differential operators on the circle, preserving the principal gradation. Considering the central extensions of the Lie subalgebra of the above algebra, fixed by these involutions, the authors classify all quasifinite (i.e. with finite dimensional \(\mathbb{Z}\)-graded pieces) highest weight representations of this algebra and find all unitary ones. The central extension mentioned above is the main object of the study in the paper. There is a realization given for the quasifinite modules in terms of highest weight representations of the central extension of the Lie algebra of infinite matrices with finitely many non-zero elements over \(\mathbb{C}[u]/(u^{m+1})\) and its classical Lie subalgebras. Further the authors obtain character formulae for positive primitive representations and realize a class of primitive representations in terms of free fields. There is a number of duality results between these primitive representations and finite-dimensional irreducible representations of finite-dimensional Lie groups and supergroups. Finally, the vacuum module of the considered algebra carries a vertex algebra structure and there is a relationship established between this module (with a half-integer parameter) and \(\mathcal{W}\)-algebras.
