Let \(Q\), \(D\), \(SD\) be a generalized quaternion 2-group, dihedral 2-group, semi-dihedral 2-group, each of order \(2^{n+1}\) (with \(n\geq 2\), \(n\geq 2\), \(n\geq 3\), respectively). Suppose \(\mathcal G\) is 2-group containing a subgroup \(H\) of index 8 (the cases of index 2 and 4 were considered earlier) where \(H\cong Q\), \(D\) or \(SD\). Then, if \(\varphi\in\text{Irr}(H)\) is faithful and \(\varphi^{\mathcal G}\in\text{Irr}({\mathcal G})\), the structure of \(\mathcal G\) is completely unraveled and determined here.
