The general structure theory of bilinear forms, formulated by \textit{C. Riehm} [J. Algebra 31, 45-66 (1974; Zbl 0283.15016)] and \textit{R. Scharlau} [Math. Z. 178, 359-373 (1981; Zbl 0473.10019)], is applied over an algebraically closed field of characteristic \(\neq 2\) to produce explicit representatives for the equivalence classes of forms, or what is the same, for congruence classes of \((n\times n)\)-matrices.
