    This paper is devoted to a proof that the set of diffeomorphism classes relative to the boundary of smooth \(h\)-cobordisms between two given closed oriented 1-connected 4-manifolds is isomorphic to the set of isometries between the intersection forms of the manifolds. A topological version is also proved.
