The structure of a finite group with exactly two conjugacy classes of real elements was described by \textit{S. Iwasaki} [in Arch. Math. 33, 512-517 (1980; Zbl 0433.20014)]. In particular, such groups have a normal Sylow \(2\)-subgroup. In the present paper, the authors study the structure of finite groups with exactly two real valued irreducible Brauer \(p\)-characters. Unlike the earlier result mentioned above, their proofs use the Classification of Finite Simple Groups. For \(p\) odd, they prove that \(G\) satisfies the condition if and only if \(G/O_p(G)\) has exactly two conjugacy classes of real elements. For \(p=2\) and \(G\) non-solvable, they obtain that \(G\) satisfies the condition if and only if \(G/O_p(G)\simeq\text{PSL}_2(7)\times K\), where \(K\) is a group of odd order.
