The authors are showing that the hypersurface in \(\mathbb{C}^5\) given by the equation \[ p=u^mv+ {(xz+1)^k- (yz+1)^l+z \over z}=0, \] with \(m\geq 2\), \(k>l \geq 3\), \(\text{gcd} (k,l)=1\) is a smooth affine variety diffeomorphic to \(\mathbb{C}^3\). As a corollary, it is shown that Miyanishi's characterization of the affine 3-space does not hold in dimension 4.
