This paper is devoted to the basic theory of star Hopf algebras and twisted star Hopf algebras. A twisted star Hopf algebra is a Hopf algebra which is simultaneously a \(*\)-algebra, and such that \(\Delta\circ*=(*\otimes*)\circ\Delta^{\text{op}}\); in contrast with the star Hopf algebra case, where \(\Delta\circ*=(*\otimes*)\circ\Delta\). Relations with invariant scalar products on modules or module-algebras, and invariant integrals are discussed. The example of the ``small'' quantum group \({\mathfrak u}_q(s\ell(2))\) at a root of 1 \(q\) is thoroughly studied. Physical motivations are mentioned.
