The author surveys the connection between isomonodromy deformation of linear systems of differential equations (mainly of Fuchsian ones) and the Painlevé equations (mainly the sixth Painlevé equation) in the sense that the sixth Painlevé equation can be derived as the Lagrange equation corresponding to an appropriate Hamiltonian system associated with Schlesinger equations.
