Let \(A\) be a von Neumann algebra (with identity element \(I\)) acting on a complex Hilbert space and let \(A_{sa}\) denote the set of Hermitian elements in \(A\). The authors prove that, if \(\Phi\) is a surjective mapping on a \(A\) satisfying \[ \Phi(T)\Phi(P)+\Phi(P)\Phi(T)^*=TP+PT^* \] for all \(T\in A\) and every element \(P\in A\) for which \(P^2=P=P^*\) (that is, a projection), then \(\Phi(I)\) belongs to the center \(Z(A)\) of \(A\) and there exists a map \(f:A_{sa}\to Z(A)\) such that \(f(s)\) is a self-adjoint element in \(Z(A)\) with \(\Phi(s)= s\Phi(I) +i f(s)\) for all \(s\in A_{sa}\). Moreover, \(f(s)=(\Phi(s)-\Phi(s)^*)/(2i)\). If, additionally, \(Z(A)\) is trivial, then there is a map \(\tau:A\to\mathbb R\) such that either \(\Phi(T)=T+i\tau(T) I\) or \(\Phi(T)=-T+i \tau(T) I\) for all \(T\in A\).
