The paper concerns the Cauchy problem \[ 2iu_t+ u_{xx}+| u|^4u= 0,\quad u(0,x)= u_0\in H^1(\mathbb{R}). \] The author exhibits a class of initial data leading to the blow-up solutions which have at least two \(L^2\)-concentration points. The mentioned class is specified by certain explicit requirements on the shape and on the energy. Then the unique solution \(u(t,x)\in C([0,T], H^1(\mathbb{R}))\) exists for a maximal \(T\) \((0<T<\infty)\) and satisfies \[ | u(t,x)|^2 dx\to \sum^L_{j=1} C_j(\delta_{p_j}(dx)+ \delta_{-p_j}(dx))+ \mu(dx) \] as \(t\to T\) in the weak topology of measures. Here \(L\geq 1\), \(p_j\) are appropriate points, \(C_j\) are positive constants, and the measure \(\mu\) is not a finite sum of Dirac measures.
