The authors study an impulsive neutral delay differential equation system \[ \begin{cases}\dot{x}(t)=D(t)\dot{x}(t-h)+f(t,x(t),x(t-h),x(t-1-h\varphi(t))),&t\not=t_i;\\ \Delta x(t_i)=I_i(x(t_i), x(t_i-h))\end{cases} \tag{1} \] with impulses at fixed instants, with a small delay \(h\) in the argument of the derivative, and with another delay \(1+h\varphi(t)\) which differs from the constant 1 by an \(\omega_1\)-periodic perturbation of a small amplitude, where \(D\) is \(\omega\)-periodic and \(f\) is \(\omega\)-periodic with respect to the first variable \(t\). It is proved that, if \(\omega\) and \(\omega_1\) are rationally independent and the so-called generating system (i.e., (1) with \(h=0\)) has an isolated \(\omega\)-periodic solution \(\psi(t)\), then, under a nondegeneracy assumption, in any sufficiently small neighborhood of the orbit \(\psi(t)\) the perturbed system (1) has a unique almost periodic solution.
