      Given an operator algebra \(A\) acting on a Hilbert space \(H\), one can consider \(H\) as a left \(A\)-module with respect to the multiplication \(a\cdot h = a(h)\), \(a \in A\), \(h \in H\). If the algebra \(A\) is self-adjoint, the problem of the flatness and the problem of injectivity of this module are equivalent. Problems concerning the spatial flatness and the spatial injectivity for some classes of non-self-adjoint operator algebras were studied in \textit{Y. O. Golovin} [Mat. Zametki, 63, No.1, 9-20 (1998; Zbl 0915.47034)], where examples of spatially non-flat and non-injective such algebras were given. However, within the class of \(C^*\)-algebras, the problem of existence of such examples remained open. In the present paper, the author constructs a von Neumann algebra which is non-flat and non-injective, that gives an answer to a question of A.Ya. Helemskii.
