The authors classify the caps \(K\) in \(\Pi=PG (r,q)\) for which the group of automorphisms of \(\Pi\) fixing \(K\) acts transitively on both \(K\) and the complement of \(K\). They obtain six cases, five of which are completely specified: (1) elliptic quadrics in \(PG(3,q)\), where \(q\) is even or an odd square; (2) Suzuki-Tits ovoids; (3) hyperovals in \(PG(2,4)\); (4) an 11-cap in \(PG(4,3)\) with automorphism group isomorphic to the Mathieu group \(M_{11}\); (5) the complement of a hyperplane in \(PG(r,2)\); (6) unions of Singer orbits. We note that the final case has not yet been classified. The essential tool in this investigation is Liebeck's classification of the affine permutation groups of rank 3.
