Let \((A, \mathfrak m)\) be a Noetherian local ring and \(I\) an \(\mathfrak m\)-primary ideal. The notion of mixed multiplicity \(\text{e}_j(\mathfrak m| I)\) is an extended one of multiplicity to a pair of ideals \((\mathfrak m, I)\). If \(A\) is Cohen-Macaulay, then there is an inequality \[ e_{d-1}(\mathfrak m| I) \geq \mu(I) - \dim A + 1 \] where \(\mu(I)\) denotes the minimal number of generators of~\(I\). The authors studied the ideal~\(I\) if equality holds Cohen-Macaulay fiber cones [\textit{C. D'Cruz, K. N. Raghavan} and \textit{J. K. Verma} in: Proc. Conf. Algebraic Geometry, Commutative Algebra, and Computation, Hanoi 1996, 233--246 (1999; Zbl 0945.13015)]. In the present paper, the authors study ideals \(I\) such that \(e_{d-1}(\mathfrak m| I) = \mu(I) - \dim A + 2\). They compute the Hilbert series of the fiber cone of~\(I\) and give a criterion for the fiber cone to be Cohen-Macaulay.
