This is a sequel to a previous paper by the authors [Invent. Math. 203, No. 2, 615--654 (2016; Zbl 1381.53113)], where the authors defined a notion of mean curvature flow with surgery for embedded mean convex surface in \(\mathbb R^3\) and showed that there exists a mean curvature flow with surgeries starting from a given closed, embedded surfaces which terminates after finitely many steps. In the present paper, the authors extend this construction to the case when the ambient space is a Riemannian three-manifold. They show that there exists a surgical mean curvature flow on any given bounded time interval. The proof uses the surgery algorithm from [the authors, loc. cit.; Invent. Math. 175, No. 1, 137--221 (2009; Zbl 1170.53042)]. Moreover, inspired by \textit{B. White}'s work [J. Am. Math. Soc. 13, No. 3, 665--695 (2000; Zbl 0961.53039)], the authors are able to give a precise description of the long-time behavior of the mean curvature flow with surgery.
