Let \(k\) be a real quadratic field and \(p\) an odd prime which splits in \(k\). Assume that the class number of \(k\) is prime to \(p\) and the fundamental unit \(\varepsilon\) of \(k\) satisfies \(\varepsilon^{p-1}\equiv 1\pmod{p^ 2}\). Denote by \(L\) the maximal \(p\)-extension of \(k\) unramified outside \(p\). Making use of Iwasawa theory, the author determines explicitly the Galois group \(G=\text{Gal}(L/k)\). As a corollary he obtains the structure of \(\text{Gal}(K/k)\), where \(K\) is the so-called field of class two of \(L/k\), i.e., the intermediate field defined by \(\text{Gal}(L/K)\cong ((G,G),G)\). Here the notation \((G_ 1,G_ 2)\) stands for the topological commutator group of \(G_ 1\) and \(G_ 2\). The article extends previous work by \textit{H. Koch} [Algebr. Number Fields, Proc. Symp. Lond. Math. Soc., Univ. Durham 1975, 609--624 (1977; Zbl 0358.12008)] in the sense that \(G\) has one unknown relation while in the similar problem treated by Koch all relations were known.
