    In this note the authors prove a conjecture from the book [\textit{M. E. H. Ismail}, Classical and quantum orthogonal polynomials in one variable. With two chapters by Walter Van Assche. Cambridge: Cambridge University Press (2005; Zbl 1082.42016), Conjecture 24.7.8], namely: Let \((P_n)_{n\ge 0}\) be a monic orthogonal polynomial sequence and \(\pi\) be a polynomial of degree at most 2 which does not depend on \(n\). If \((P_n)_{n\ge 0}\) satisfies \[ \pi(x){\mathcal D}_q P_n(x) = (a_nx + b_n)P_n(x) + c_nP_{n-1}(x), \] then \((P_n)_{n\ge 0}\) are the continuous \(q\)-Jacobi polynomials, Al-Salam-Chihara polynomials, or special or limiting cases of them. The proof of this statement is summarized in the following two ones: \begin{itemize} \item The Al-Salam Chihara polynomials with nonzero parameters \(c\) and \(d\) such that \(c/d = q^{\pm 1/2}\) are the only orthogonal polynomial sequence satisfying such condition for \(\deg \pi = 1\). \item The Chebyschev polynomials of the first kind and the continuous \(q\)-Jacobi polynomials are the only orthogonal polynomial sequence satisfying such condition for \(\deg \pi= 2\). \end{itemize}
