The paper is devoted to classification of binary quadratic form that have unique (up to isometries) representation as a sum of \(n\) integral squares (\(n\geq 4\)). For \(n=4\) and \(n=5\) the number of such forms is infinite and way are classified. For \(n\geq 6\) the number of such forms is finite (10 for \(n=6\), 8 for \(n=7\) and 6 for \(n\geq 8\)). Moreover, quadratic forms \([1,0,1]\), \([1,0,2]\), \([1,0,3]\), \([2,1,2]\), and \([2,1,3]\) are uniquely represented as a sums of \(n\) integral squares for each \(n\geq 4\) and the form \([2,1,4]\) is uniquely represented as a sums of \(n\) integral squares for each \(n\geq 5\).
