    In the article, the authors continue (see [\textit{S. Yu. Slavyanov} and \textit{A. A. Salatich}, J. Math. Sci., New York 232, No. 2, 157--163 (2018; Zbl 1397.34154); translation from Zap. Nauchn. Semin. POMI 462, 93--102 (2017)]) to study the relationship between the Heun confluent equations \[ (H(z,D)-h)w(z):=(z^2 D^2+(-z^2 -tz+c)D+(-az-h))w(z)=0,\tag{1} \] \[ (H(z,D)-h)w(z):=(D^2 +(-z^2 -t)D+(-az-h))w(z)=0,\tag{2} \] and Painlevé equations. Equations (1) and (2) are considered for which, using the antiquantization process, it is possible to obtain Painlevé equations P4 and P2, respectively. The same Painlevé equations can be obtained by replacing equations (1) and (2) by the corresponding systems of linear differential equations and considering the isomonodromic deformations of these systems. The authors derive the system \[ Y'=(A_1/z+A_2+A_3z)Y, \] where \(A_1 = \begin{pmatrix} 0 & 0 \\ h/t & -c \end{pmatrix}\), \(A_2=\begin{pmatrix} 0 & t \\ (a-c+1)/t & t \end{pmatrix}\), \(A_3=\begin{pmatrix} 1 & 0 \\ 1 & 0\end{pmatrix}\), for the first equation and the system \[ Y'=(A_1 +A_2 z+A_3 z^2)Y, \] where \(A_1=\begin{pmatrix} 0 & 1\\h & t\end{pmatrix}\), \(A_2=\begin{pmatrix} 0 & 0 \\ a & 0 \end{pmatrix}\), \(A_3=\begin{pmatrix} 1 & 0 \\ 1 & 0 \end{pmatrix}\), for the second one. Note that the wild monodromy is implied, since the usual monodromy for the second equation is trivial.
