In connection with the study of quadrature domains for harmonic functions, \textit{B. Gustafsson, M. Sakai} and \textit{H. S. Shapiro} [Potential Anal. 7, No. 1, 467--484 (1997; Zbl 0882.31002)] posed the following question: are the integrable positive harmonic functions on a bounded domain dense in the cone of all positive harmonic functions, in the topology of uniform convergence on compact sets? The author characterizes domains with this approximation property (and the analogous property where ``integrable'' is replaced by ``bounded'') in terms of Martin boundary theory. He then combines his general results with an idea of \textit{A. Ancona} [Ann. Inst. Fourier 29, No. 4, 71--90 (1979; Zbl 0589.31002)] to construct nontrivial counterexamples to questions of the type mentioned above.
