Homogeneous affine \(n\)-dimensional hypersurfaces are important examples of hypersurfaces with the property that the affine differential geometric concepts (affine metric, affine normal vector, affine shape operator, etc..) calculated at one point can be taken to any other point by an affine transformation of the \((n+1)\)-space. In dimension 2, there is a complete classification of locally strongly convex, locally homogeneous, affine hypersurfaces [\textit{H. W. Guggenheimer}, Differential geometry. 2nd ed. New York: Dover Publications, Inc (1977; Zbl 0357.53002); \textit{K. Nomizu} and \textit{T. Sasaki}, Manuscr. Math. 73, No. 1, 39--44 (1991; Zbl 0753.53008)] and the same happens in dimension 3 [\textit{F. Dillen} and the second author, Manuscr. Math. 80, No. 2, 165--180 (1993; Zbl 0795.53008)]. In dimension 4 there is classification of those hypersurfaces which are quasi umbilical, i.e., the shape operator has only two distinct eigenvalues, one of them with multiplicity one [\textit{F. Dillen} and the second author, J. Math. Soc. Japan 46, No. 3, 477--502 (1994; Zbl 0820.53009)]. In this paper, the authors study locally strongly convex locally homogeneous hypersurfaces of dimension 4 whose shape operator has two distinct eigenvalues, each of multiplicity two. They show that there are only three such hypersurfaces: Two of them are generalizations of Calabi products [\textit{F. Dillen} and the second author, Differ. Geom. Appl. 4, No. 4, 303--328 (1994; Zbl 0934.53006)], but the other is obtained by a new construction.
