    Let \(A\) be a commutative Noetherian ring in which \(2\) is invertible and let \(B\) be the polynomial \(A\)-algebra \(A[X_1, \ldots, X_n]\) in \(n\) indeterminates. Let \(Q = (Q,q)\) be a quadratic space over \(B\) and let \(Q_0 = (Q_0,q_0)\) be the reduction of \(Q\) modulo the ideal of \(B\) generated by \(X_1, . \ldots, X_n\). \textit{A. A. Suslin} and \textit{V. I. Kopejko} proved in [Zap. Nauchn. Semin. Leningr. Otd. Mat. Inst. Steklova 71, 216--250 (1977; Zbl 0416.20045)] that if \(Q\) is stably extended from \(A\) and for every maximal ideal \(\mathfrak{m}\) of \(A\), the Witt index of \(A_{\mathfrak{m}} \otimes A (Q_0,q_0)\) is larger than the Krull dimension of \(A\), then \((Q,q)\) is extended from \(A\). It was shown by the second author in [J. Algebra 86, 159--180 (1984; Zbl 0537.10013)] and [J. Algebra 89, 88--101 (1984; Zbl 0542.10014)] that one can improve this result to Witt index \(d\), when \(A\) is a local ring at a non-singular point of an affine variety of dimension \(d\) over an infinite field. Moreover, a question was posed whether extendability can be shown for quadratic spaces with Witt index \(d\) over polynomial extensions of any equicharacteristic regular local ring of dimension \(d\). In the article under review the authors establish this question affirmatively.
