The authors consider perturbed (generalized) Dirac operators on even-dimensional open manifolds with boundary; here, the manifolds and bundles are assumed to be of bounded geometry (their doubles are of bounded geometry), the (possibly non-compact) boundary is assumed to have a collar neighborhood of uniform width, and the perturbations of the Dirac operators are given by potentials (bundle maps) satisfying a list of assumptions concerning its growth and regularity at infinity. It is shown that these assumptions can be used instead of compactness to guarantee that such operators have discrete spectrum with polynomial growth, and that the index theorem can be generalized to this setting with essentially the same proof and without changing the Atiyah-Patodi-Singer index formula [\textit{M. F. Atiyah, V. K. Patodi}, and \textit{I. M. Singer}, Math. Proc. Camb. Philos. Soc. 77, 43--69 (1975; Zbl 0297.58008)]. It is also shown that the needed assumptions hold when there are finitely many ends which are cylindrical or conically expanding. The empty boundary case was considered by the same authors in [Int. J. Math. 14, 69--104 (2003; Zbl 1079.58019)].
