      The author investigates the extremal ranks and inertias of the Hermitian matrix expression \(f(X, Y ) = C_4 - B_4Y - (B_4Y )^*-A_4XA_4^*\), where \(C_4\) is Hermitian and \(X\) and \(Y\) satisfy the following consistent system of matrix equations \(A_3Y =C_3\), \(A_1X = C_1\), \(XB_1 = D_1\), \(A_2XA_2^*=C_2\), \(X=X^*\). As a consequence, some necessary and sufficient conditions for the above expression \(f(X, Y )\) to be (semi)positive and (semi)negative are obtained. Moreover, some necessary and sufficient conditions for the solvability of the system of matrix equations \(A_3Y = C_3\), \(A_1X = C_1\), \(XB_1 = D_1\), \(A_2XA_2^*=C_2\), \(X=X^*\), \(B_4Y+(B_4Y)^*+A_4XA_4^*=C_4\) are given and an expression of the general solution to this system, when it is solvable, is provided. As a special case of this system, some new solvability conditions for the system \(A_1X = C_1\), \(XB_1 = D_1\), \(A_2XA_2^* = C_2\), \(A_4XA_4^*=C_4\) and a new expression of its general solution are presented.
