Chain approximations, introduced in \textit{H.-W. Siegberg} and \textit{G. Skordev}, Pac. J. Math. 102, 455-486 (1982; Zbl 0458.55001) for m-acyclic maps [\textit{G. Conti}, \textit{J. Pejsachowicz}, Ann. Mat. Pura Appl., IV. Ser. 126, 319-341 (1980; Zbl 0463.55003)] of finite polyhedra are constructed. As a corollary a fixed point index with all properties, i.e., homotopy invariance, additivity, normalization, commutativity and mod p property, for these maps is obtained. This generalizes the results of \textit{C. N. Maxwell} [Proc. Amer. Math. Soc. 8, 808-815 (1957; Zbl 0078.15301)], \textit{S. Masih} [Fundam. Math. 102, 143-158 (1979; Zbl 0401.55003)], \textit{H. Schirmer} [Contemp. Math. 21, 207-212 (1983; Zbl 0528.55004)], and \textit{Z. Dzedzej} [Diss. Math. 253 (1985; Zbl 0592.55001)]. A unified approach to the Borsuk-Ulam theorem and its generalizations by Bourgin-Yang for multivalued maps admitting chain approximations is developed. As a corollary these theorems are proved for m-acyclic maps. This generalizes similar results of \textit{H. Schirmer}, loc. cit., and \textit{K. Gȩba} and \textit{L. Gorniewicz} [Bull Pol. Acad. Sci., Math. 34, 315-322 (1986; Zbl 0613.55001)].
