    This paper is a continuation of the authors work [Trans. Am. Math. Soc. 347, No. 6, 2255--2262 (1995; Zbl 0853.53047); J. Geom. Anal. 12, No. 1, 63--79 (2002; Zbl 1030.58006); Int. J. Math. 14, No. 3, 259--287 (2003; Zbl 1048.32014); Ann. Global Anal. Geom. 30, No. 2, 139--167 (2006; Zbl 1098.32010)] and [the author and \textit{X. Chen}, Asian J. Math. 4, No. 4, 817--830 (2000; Zbl 1003.32003)] studying the existence of extremal Kähler metrics on almost-homogeneous manifolds of cohomogeneity one. While previously the author focussed on the type I and type III cases, the current paper in conjunction with [the author, Cent. Eur. J. Math. 7, No. 1, 84--123 (2009; Zbl 1176.53073)] focusses on the type II case with hypersurface ends. Among other results, the paper proves that there exists a constant scalar curvature Kähler metric on type II almost-homogeneous manifolds whose generalized Futaki invariant is positive (see Theorem 15) and where the definition of the generalized Futaki invariant used by the author here is different from the one in [\textit{W. Ding} and \textit{G. Tian}, Invent. Math. 110, No. 2, 315--335 (1992; Zbl 0779.53044)]. The author also obtains sufficient conditions for the existence of Kähler-Einstein metrics on Fano non-affine type II cohomogeneity one manifolds (Theorem 15') as well as necessary and sufficient conditions for the existence of Kähler-Einstein metrics on comogeneity-one, two-orbit Fano manifolds with a semisimple group action (Theorem 24).
