      Suppose \(M\) is a compact irreducible sufficiently large 3-manifold with incompressible boundary containing 2-sided projective planes. We describe a finite subgroup of the automorphism group, isomorphic to \(\oplus{\mathbb{Z}}_ 2\), such that for any automorphism \(\phi\) of the fundamental group, there exists an element \(\alpha\) in \(\oplus{\mathbb{Z}}_ 2\) so that their composition \(\alpha\phi\) is realizable by a homeomorphism. Using this we show that the subgroup of realizable automorphisms has finite index in the automorphism group, and that this index is less than or equal to two when every 2-sided projective plane separates.
