This paper studies the explicit expression of the higher-order differential operator associated with the Koornwinder generalized Jacobi polynomials. These polynomials are orthogonal with respect to a classical Jacobi weight on $[-1,1]$ (with two parameters $\alpha,\beta>-1$) adding two Dirac deltas at the endpoints of the orthogonality interval ($-1$ and $1$) with masses given by two other parameters $M,N\geq0$. For $\alpha$ and $\beta$ nonnegative integers, the order of this differential operator is $2\alpha+2\beta+6$. This differential operator is presented here as a linear combination of four elementary components, showing the symmetry with respect to the underlying scalar product and thus verify the orthogonality of the eigenfunctions.
