An example of a Banach lattice satisfying the properties of the title is given. The space is constructed starting with an increasing sequence of finite-dimensional subspaces of the separable reflexive Banach lattice without the approximation property given in [\textit{A. Szankowski}, Isr. J. Math. 24, 329--337 (1976; Zbl 0336.46013)] and careful renorming arguments. We refer to the paper for details.
