    Let \(E\) be a Banach lattice and \(F\) be a Banach space. A bounded linear operator \(T:F\rightarrow E\) is called almost surjective if, for all \(0\leq y\in E\), there exists \(x\in F\) such that \(y\leq Tx\). A linear operator \(T:E\rightarrow F\) is called a cone isomorphism if there exist constants \(C_1,C_2\) with \(C_1 \|x\| \leq \|Tx\| \leq C_2 \|x\|\) for all \(0\leq x\) in \(E\). If \(C_1 = C_2 = 1\), then \(T\) is called a cone isometry. It is proved that a bounded linear operator \(T:E\rightarrow F\) is a cone isomorphism if and only if the adjoint \(T^*\) is almost surjective. Then the dual result that a bounded linear operator \(T\) from \(E\) into \(F\) is almost surjective if and only if the adjoint \(T^*\) is a cone isomorphism is proved. The author then considers positive linear operators from \(L_p\) spaces into Banach lattices and studies the relation between positive cone isomorphisms (isometries) and positive isomorphisms. It is proved that, if \(0\leq T:L_p\rightarrow E\) is a cone isometry, with \(1 < p < \infty\) and \(E\) having strictly monotone norm, then \(T\) is a lattice isometry. Finally, applications are given for optimal domains of positive operators.
