\textit{Yu. G. Prokhorov} [Sb. Math. 196, No. 1, 77--114 (2005; Zbl 1081.14058)] proved that a Fano threefold \(X\) with canonical Gorenstein singularities has anticanonical degree \((-K_X)^3\leq 72\) and if the equality holds, then \(X\) is isomorphic to either \(\mathbb{P}(3,1,1,1)\) or \(\mathbb{P}(6,4,1,1)\). In a previous work [Sb. Math. 200, No. 8, 1215--1246 (2009); translation from Mat. Sb. 200, No. 8, 111--146 (2009; Zbl 1185.14036)], the author of the present article gave a complete classification of Fano threefolds whose anticanonical degree is between \(64\) and \(72\). The main result of this article is a complete classification of Fano threefold \(X\) with anticanonical degree \(64\). The proof goes via a careful study of all positive Mori contractions on a terminal \(\mathbb{Q}\)-factorial modification of \(X\).
