For a closed connected oriented \(4\)-manifold \(M\), let us write \((\pi ,\pi_2, \Lambda) =(\pi_1(M),\pi_2(M),\mathbb Z [\pi])\), and let \(\text{Aut}_{\bullet}(M)\) denote the group of based homotopy classes of orientation preserving self-homotopy equivalences over \(M\). Let \(w_2=w_2(M)\in H^2(M,\mathbb Z/2)\cong [M,K(\mathbb Z/2,2)]\) denote the second Stiefel-Whitney class of \(M\) and let \(\text{Aut}_{\bullet}(M,w_2)\) be the thickening of \(\text{Aut}_{\bullet}(M)\) defined by Hambleton and Kreck in [\textit{I. Hambleton} and \textit{M. Kreck}, Math. Z. 248, No.~1, 147--172 (2004; Zbl 1068.57018)]. Note that there is a short exact sequence \[ 0\to H^1(M,\mathbb Z/2)\to \text{Aut}_{\bullet}(M,w_2)\to \text{Aut}_{\bullet}(M)\to 1. \] Similarly, let \(\text{Isom}[\pi,\pi_2,k_M,c_*[M]]:=\{\phi \in \text{Aut}_{\bullet}(B):\phi_*(c_*[M])=c_*[M]\}\) and let \(\text{Isom}^{\langle w_2\rangle}[\pi,\pi_2,s_M]\) denote the extension of \(\text{Isom}[\pi,\pi_2,k_M,c_*[M]]\) defined by the author, where \(B\) is the \(2\)-type of \(M\) and \(s_M\) (resp. \(k_M\)) denotes the intersection form on (resp. the \(k\)-invariant of \(M\)). In this paper, the author studies the group \(\text{Aut}_{\bullet}(M,w_2)\) and he proves that there is an isomorphism \[ \text{Aut}_{\bullet}(M,w_2)\cong (KH_2(M,\mathbb Z/2)\oplus H_3(M,\mathbb Z/2)) \rtimes \text{Isom}^{\langle w_2\rangle}[\pi,\pi_2,s_M]. \] The proof is based on the braid of exact sequence given by Hambleton and Kreck and the computations of modified bordism groups [\textit{I. Hambleton} and \textit{M. Kreck}, Math. Ann. 280, No.~1, 85--104 (1988; Zbl 0616.57009)].
